• Application forms for entrance examination will be available at the selected branches of State Bank of India on payment of Rs. 750/- ( Rs. Seven Hundred and fifty only) through cash.

• Application forms are also available at all participating colleges on payment of Rs. 750/- ( Rs. Seven hundred and fifty only ) through cash/ demand draft in favour of APDMC payable at Bhopal.

• Application Forms can also be obtained from the Association Office/Participating College office by post by remitting a DD of Rs.850/- (including postal charges)
Application form is also available on website of Association (www.mpapdmc.com) and the downloaded from this site. In case the application form has been downloaded from website it shall be submitted alongwith Demand Draft of Rs. 1500/- drawn in favour of APDMC, Bhopal.

Before filling the application form the candidate must familiarize himself/herself with the information given in this brochure and shall satisfy his/her eligibility to appear in the examination. The candidate is required to go through the information brochure carefully and acquaint himself/herself with all requirements with regard to the filling-in of the Application Form. Application Form will be filled by Black Ball Point Pen in English only.
The candidate while filling up the form must indicate the order of courses i.e. MBBS, BDS and also the preferences of college.

Overwriting, cuttings, erasing in the application forms may lead to rejection of the form and should be avoided. Any error arising on this account shall be the responsibility of the candidate. (It is advisable to first fill in a photocopy of the form and then fill in the main document.) Candidate should write his/her complete address in the application form for further correspondence. Pin Code should be given in the space provided for Pin Code. Candidates are also advised to keep photocopy of OMR Form in his record.

• The Application Forms complete in all respect should be submitted to the Secretary, APDMC, DMAT OFFICE, E-2/51, Arera Colony, Near Niramay Hospital, Opp. Habibganj Railway Station, Bhopal 462016 by Registered / Speed Post/Courier/in person with Examination Fee of Rs. 750/- by Demand Draft drawn in favour of APDMC, payable at Bhopal latest by 10th May 2007.

• The application is required to be duly returned in the envelope provided alongwith the Application Form.

• Incomplete Application Forms will be rejected.

Note: Form should be filled by the candidate in his/her own handwriting. It is suggested that candidate make a photocopy of the form and use that for practice.

Please fill the form with great care, requests for corrections will not be entertained at any stage incompletely filled application will be rejected without any notice. Candidates should read all the instructions carefully and fill the form with utmost clarity. They should use black ball pen only to write in the boxes and to darken the circles in the columns/lines provided.

5.1 Name of the: Write first name, then middle name (abbreviate if necessary) and then surname in the boxes provided on top of the circles. Leave a box blank between first, second and surname. Darken the corresponding circles with Black Ball Pen

5.2 Sex: Write Male or Female in the box provided and darken the appropriate circle.

5.3 Date of Birth of Candidate : In the first column two boxes are provided; write the date in the boxes one box is provided. Write the month (January as Jan) and darken the corresponding circle. In the third column two boxes are given. Write the year (if born in year 1991 write 9 and 1) in the boxes and darken the corresponding circles.

5.4 Choice of Centres : Pick up the choice of the two centres in order of preference and darken the corresponding circles in FIRST & SECOND choice.

5.5 A recent colour high contrast 3.5 cm. x 4.5 cm. size photograph is required. Only photograph of the front profile with full face, head, shoulder and with both ears visible and taken without wearing any cap or hat or sunglasses will only be accepted. Polaroid photo is NOT acceptable. Photograph MUST be taken on or after 1st March 2007 with a placard indicating the name of the candidate(as in the application form) and the date of taking photograph.

In case name and date are written on the photograph after taking it, the application will be rejected.
The name and date on the photograph must be clear and legible.

Photograph should be pasted in such a manner that full profile or head, face and shoulder lies entirely inside the box and no where else. Do not use pin/staple to fix the photograph. It is not necessary to attest or sign the photograph.

5.6 Mailing address should be clearly written entirely inside the box. Write your name, followed by the address and finally PIN code.

Write name and address in capital letters only.

5.7 Signature should be made by the Candidate himself/herself entirely inside the box.

5.8 If you have contact phone number please fill in the boxes.

5.9 If you have e-mail address please fill in the box.

5.10 Name of Parent/Guardian: Write first name, then middle name (abbreviate if necessary) then surname in the boxes provided. Leave a box blank between names. Darken the corresponding circles.

5.11 Darken the circle to indicate the guardian’s relationship with you.

NOTE :

(a) All the particulars in the application OMR Form must be written carefully and legibly by the candidate in his/her own hand writing.

(b) Do not tamper the Bar code of application form. Any attempt will lead to rejection of form.

(c) A singly recent 3.5 cm. X 4.5 cm. size unattested photograph of the candidate should be pasted in the space, provided in the OMR Form entirely within in the margins of the box. In no case, application form with pinned/stapled photograph shall be accepted. Xerox of Photograph shall NOT be accepted.

(d) Any attempt on the part of a candidate to influence the authorities, directly or indirectly will be treated as disqualification of candidature for admission to MBBS/BDS courses.

(e) The candidates are advised to keep the photocopy of OMR application form (of both sides)

6. FOR FUTURE CORRESPONDENCE:

(a) Mention your address for communication in the appropriate column.

(b) Thereafter no change will be permitted and the candidate will have to make his/her arrangement for receiving the mail on that address or redirecting of the mail.
(c) The APDMC shall not take any responsibility for non- delivery of any communication on above account.

(d) Always mention your application form number as printed on top right hand corner of the form. After the allotment of roll number the candidate should mention the Roll Number in all the correspondence.

The NRI candidates shall apply directly to the Head of the concerned Institution in the application form prescribed by the Institution along with the following certificates/ documents:-

Mark Sheet of the qualifying examination.
Eligibility Certificate.
Certificate issued by Indian Diplomatic Mission or Chancellery or Commission abroad under its seal stating that the father/parent of the applicant or applicant himself/herself is an Indian Resident in that Country.

The screening of the candidates shall be done at the institution level. The admissions shall be done on the basis of merit list prepared from the applications received. If the seats are not filled under NRI quota then such seats shall be filled as drop out / vacant seats through DMAT Merit. In such cases college shall inform the detail of vacant seats in writing by 25.09.2007 (for admission through DMAT 2007 to the office of the Judges Committee for Monitoring Procedures (Medical) as well as to APDMC office) so that remaining seats can be filled through DMAT Merit before the last date of admission 30.09.2007.
